Report Card

 All co-pays and balances are due at the time of service.

 All Private Pay patients are expected to pay at the time of service.

 Please note we ask that our patients give us 24 hour notice for cancelled or rescheduled appointments. A $25 charge will be applied if appropriate notice is not given.
